I don't think we have documentation yet as our integration has yet to reach its final form.
I can, however, help you with the configuration.

My Proxmox config looks like this. (Url was previously Host but the expected value is the same)

In your case, the Realm will be a textbox. You can either leave it empty if you connect using Linux PAM or use 'pve' if you select Proxmox VE when logging through the web interface.
Username and password should be the same you input in the web interface.
