User Vault keeps duplicating some entries
They say a picture is worth a thousand words. This started right after the upgrade to 2021.1.15.0 64-bit

Thank you for reporting the issue. We had another client report the issue a few days ago and have been working on a fix. It is now resolved internally and will be available in the next beta release.
You will need to manually clean the duplicated entries but then it should stop creating new duplicates.
