SSH - "Disconnect" indicator lost

In some circumstances, the "Disconnect" indicator written on a tab is lost. For instance, see below after splitting the tab:

There are other occurrences where I've seen the "Disconnect" indicator disappear without splitting the tab, but I don't know how to reproduce it. :/
