New Template for Username/Password without template name

What version of RDM are you using?

As workaround you can create an Credential entry in your Repository (with the name that you want use for the template) and save it as template with following option:

Right Click on the entry => Hinzufügen => Als Vorlage speichern...

I tried 13.5.11.0 (zip) and the Name is still inactive. All Credential entries are affected from this behaviour.
