I can connect fine from a Windows box using the exact same credentials. I'm running version 4.1.0.0. The error says:

SQL server. I don't want to reveal any site specific information so I've changed the host, domain, and user but everything else is the same.

In the Server field, could you try to enter the IP address of the SQL Server instead of the hostname.
In the Domain field, could you just enter domain instead of domain.com.

Same results. The test buttons are successful but the error message is the same when trying to connect to the Data Source.

Correct, but on Windows when you use Integrated security it doesn't ask for user credentials like the Mac version does.

Exactly, Integrated Security is not hanlded the same way on a mac computer.
