Since running 12.0.8.0 and needing to change my NT password, the SSH Port Forward won't accept 'My personal credentials' anymore and generates the above error, but if I use embedded (username/password) then it takes them fine.

Hi,
Could you try to edit your My Personal Credential and set your credentials again?

Already did that, 3 times, same thing...

Could you try you credentials with another entry?

Yep, works fine (uses the same My Personal credential set)

Could you try to duplicate the entry and retry? It seems that something is blocking the My Personal Credentials.

Duplicating the entry results in the same error message :-(

Do you have the domain name in your working entry? Is it exactly the same format?

I had the domain name in the working entry, removed it and it's now working fine (I'm sure that used to work?!). Thank you for your help!

Have you splited the domain and the username (two fields) in My Personnal Credentials.

Yes, I have now. Previously I had [domain name][username] in the username field and the domain name in the domain field.

This could explains the problem. I know how we could fix this for another version.
